How to Prepare for BPH Surgery

Preparing for BPH laser surgery (HoLEP/ThuLEP) involves a short pre-operative workup — PSA, a prostate ultrasound, and symptom scoring — plus a review of your current medications, particularly blood thinners. Because laser enucleation carries a low bleeding risk, some anticoagulants or antiplatelet drugs can often be safely continued rather than stopped, though this decision is always made individually with your prescribing physician. You'll also be given fasting instructions before anesthesia and told what to bring on the day.

The Pre-Operative Workup

Managing Blood Thinners and Other Medications

Because laser enucleation has a lower bleeding risk than traditional resection (TURP), many patients on aspirin, clopidogrel, warfarin, or newer blood thinners (DOACs) can continue them without stopping — a real advantage for patients who can't safely pause these medications for other health reasons. That said, this is always an individual decision made jointly by your surgeon and the physician who prescribed the medication. Never stop or continue any blood thinner on your own.

Fasting and Anesthesia Instructions

You'll typically be asked to have nothing to eat or drink (NPO) for a set number of hours before surgery — your anesthesia team will give you exact timing rather than a general rule, since it can vary by case. Some regular medications are taken as usual with a small sip of water; others are paused. Your surgical team will give you a clear written list.

What to Bring on the Day

What Happens After You're Prepared

Once your pre-op evaluation is complete, surgery is typically scheduled within a short window. For what happens on the day of surgery itself and during recovery, see our full guide on what to expect after BPH laser surgery.

The Bottom Line

Preparing for BPH surgery is straightforward: a short set of pre-op tests, an individualized review of your medications (especially blood thinners), clear fasting instructions, and a simple checklist for the day itself. Most of this is coordinated for you by your surgical and anesthesia team.
